Box breathing involves four simple actions — inhale, hold, exhale, pause — but what actually happens in the body during those 16 seconds?
Normal resting breathing is approximately 12–16 breaths per minute. Box breathing at a 4-count produces about 4 breaths per minute. This slower rate is the foundation for most of the proposed effects.
Carbon dioxide is the primary trigger for the breathing urge. Anxious or fast breathing can lower CO2 slightly, which may contribute to feelings of lightheadedness or unease. Slow, deliberate breathing — particularly the hold phases — helps maintain a stable CO2 level, which many researchers propose contributes to the calming experience.
The baroreflex is a blood pressure feedback system that operates partly in sync with the breath. Slow, rhythmic breathing is proposed to enhance baroreflex sensitivity. This is one proposed pathway for why slow breathing may influence blood pressure and HRV in some studies.
Heart rate naturally rises during inhale and falls during exhale. Slow breathing amplifies this RSA pattern, which is one component of heart rate variability (HRV).
The counting in box breathing also has a psychological dimension — it occupies the mind with a simple, structured task, which may help interrupt rumination independently of physiological mechanisms.
These mechanisms are proposed based on current breathing research — not confirmed causes and effects for every individual. Breathing science is an active and still-developing field.
